Memorials and Donations

At Fairview Lakes HomeCaring & Hospice, our mission comes first...to serve each person with dignity, integrity, service and compassion. Because home care and hospice payment often falls short of the actual cost of services, we rely on financial gifts from generous donors to meet our patient and staff needs. If you’d like to be a partner in this caring service to bring peace and comfort to others, please consider a tax–deductible gift to Fairview Lakes HomeCaring & Hospice. (Contributions are used to support services within our community.)

Hospice donations and memorials provide benefits such as:

  • Financial assistance for patients without resources to pay for needed services
  • Music and massage therapy to ease anxiety and pain
  • Patient and family education materials
  • Bereavement care for family and friends
  • Training and support for hospice staff and volunteers

Home care donations and memorials provide benefits such as:

  • Financial assistance for patients without resources to pay for needed services
  • Counseling and spiritual care visits for patients with an advanced, progressive illness
  • Resources such as clinical reference manuals and resuscitation manikins

Gifts may be given in the form of memorials for loved ones who have died, tributes to staff or volunteers who helped make a difference, bequests and estate gifts or general donations in support of our mission. Checks or money orders may be sent to:

Fairview Lakes HomeCaring & Hospice
11725 Stinson Ave.
Chisago City, MN 55013

If the donation is a memorial gift, please include the name of the individual in whose memory the gift is being made.

For more information about donations, contact the hospice coordinator at 651-257-7836 or the director of HomeCaring & Hospice at 651-257-7804.
